Amendment to a significant contract in the subsidiary company Powiatowe Centrum Zdrowia sp. z o.o. in Kowary

Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodic information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. announces that on December 30th 2015 the Company received a settlement to the contract concluded by the subsidiary company Powiatowe Centrum Zdrowia sp. z o.o. with the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch) regarding healthcare services such as inpatient care as well as inpatient care care in the period 1 January to 31 December 2012. Healthcare services referred to in the said contracts are provided by the subsidiary company of EMC Instytut Medyczny S.A.- Powiatowe Centrum Zdrowia sp. z o.o. in Kowary.

The Issuer informed about the contracts concluded by Powiatowe Centrum Zdrowia sp. z o.o. in Kowary with the Fund Branch concerning rendering services in the current year (2015) in its current reports nos in 2012 in its current reports nos. 2/2012 of 20.01.2012;10/2012 of 25.01.2012; 19/2012 of 02.02.2012; 30/2012 of 02.03.2012; 47/2012 of 02.05.2012; 50/2012 of 28.05.2012; 53/2012 of 01.06.2012; 65/2012 of 21.06.2012; 67/2012 of 29.06.2012; 75/2012 of 08.08.2012; 88/2012 of 03.09.2012; 91/2012 of 06.09.2012; 99/2012 of 26.09.2012; 110/2012 of 16.11.2012; 120/2012 of 20.12.2012; 26/2013 of 28.02.2013; 32/2013 of 18.04.2013. As a result of signing the abovementioned settlement to the contract with the NHF the value of the contract with highest value has changed and it totals PLN 9,378,495.20 (an increase by PLN 43,309.20). As a result of signing the abovementioned annexes the total maximum Fund Branch liability towards the subsidiary currently amounts to PLN 7,084,160.28 (an increase by PLN 43,309.20) for the period 1 January to 31 December 2012. The other material terms of the contract with the highest value made public remain unchanged. A criterion for a contract to be considered significant: The value of contracts exceeds 10% of the Issuer’s equity capital.
