Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodical information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A., in connection with receiving as of December 28th 2015 annexes to the contract concluded with the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch) signed by the subsidiary Mikulicz sp. z o.o. regarding healthcare services in the scope of inpatient care, outpatient specialist care, outpatient specialist care (cost-consuming outpatient diagnostic services), palliative and hospice care, psychiatric care and addiction treatment as well as nursing and care services in terms of long-term care for the period 1 January to 31 December 2015. Healthcare services referred to hereinabove are provided by the subsidiary Mikulicz Sp. z o.o. in Świebodzice owned by EMC Instytut Medyczny S.A.

The Issuer informed about the contracts concluded by Mikulicz sp. z o.o. with the Fund Branch concerning rendering healthcare services in 2015 in its currents report nos. 32/2015 of 4.03.2015, 43/2015 of 24.04.2015, 47/2015 of 26.05.2015 and 53/2015 of 07.07.2015. As a result of signing the said annexes concluded with the National Health Fund regarding healthcare services provided in the period 1 January to 31 December 2015, the value of the contract with highest value has changed and it currently amounts to PLN 11,014,535.40 (an increase by PLN 119,899.20).The total maximum Fund Branch liability towards the subsidiary company currently amounts to PLN 14,723,923.80 (an increase by PLN 292,912.00) in the period 1 January to 31 December 2015. A criterion for a contract to be considered significant: The value of contracts exceeds 10% of the Issuer’s equity capital.
