Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodic information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. announces that on August 10, 2015 the Company received an annex to the contracts concluded by the subsidiary company Powiatowe Centrum Zdrowia sp. z o.o. with the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch) regarding healthcare services such as inpatient care for the period 1 January to 31 December 2015. Healthcare services referred to in the said contracts are provided by the subsidiary company of EMC Instytut Medyczny S.A.- Powiatowe Centrum Zdrowia sp. z o.o. in Kowary. The Issuer informed about the contracts concluded by Powiatowe Centrum Zdrowia sp. z o.o. in Kowary with the Fund Branch concerning rendering services in the current year (2015) in its current reports nos. 9/2015 of 20.01.2015, 52/2015 of 1.07.2015 and 57/2015 of 28.07.2015. As a result of signing the abovementioned annex to the contract with the NHF concerning rendering healthcare services, the value of the contract with the highest value has changed and it totals PLN 9,340,887.00 (an increase by PLN 10,296.00). As a result of signing the abovementioned annexes, the total maximum Fund Branch liability towards the subsidiary currently amounts to PLN 17,327,888.53 (an increase by PLN 10,296.00) for the period 1 January to 31 December 2015. A criterion for a contract to be considered significant: The value of contracts exceeds 10% of the Issuer’s equity capital.
