Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodical information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. reports that on March 16, 2015 the Company received an annex to the contract concluded with the National Health Fund - the Opolskie Voivodeship Branch in Opole (the Fund Branch) regarding healthcare services in the scope of therapeutic rehabilitation for the period 1 January to 31 December 2014. Healthcare services referred to hereinabove are provided by St. Roch's Hospital in Ozimek near Opole owned by EMC Instytut Medyczny S.A. The Issuer informed about the contracts concluded with the Fund Branch concerning services rendered in the previous year (2014) in its current reports nos. 12/2014 of 24.01.2014, 36/2014 of 28.02.2014, 50/2014 of 30.04.2014, 62/2014 of 12.06.2014, 72/2014 of 23.07.2014, 84/2014 of 16.09.2014, 88/2014 of 02.10.2014, 92/2014 of 30.10.2014, 101/2014 of 2.12.2014, 111/2014 of 22.12.2014, 5/2015 of 16.01.2015 and 36/2015 of 13.03.2015. As a result of signing the abovementioned annex, the value of the contract with the highest has not changed. The total Fund Branch liability towards the Company currently amounts to PLN 13,742,662.90 (an increase by PLN 5,245.18) for the period 1 January to 31 December 2014. A criterion for a contract to be considered significant: The value of contracts exceeds 10% of the Issuer’s equity capital.
