Report no. 2/2016

Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodical information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. informs about receiving on January 5th 2016 a settlement and annexes to the contracts concluded with the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch) regarding healthcare services in the scope of inpatient treatment, outpatient specialist care, outpatient specialist care (cost-consuming outpatient diagnostic services).

Healthcare services referred to hereinabove are provided by St. Anthony’s Hospital in Ząbkowice Śl. owned by EMC Instytut Medyczny S.A. The Issuer informed about the contracts concluded by St. Anthony’s Hospital in Ząbkowice Śl.  with the Fund Branch concerning rendering services in 2012 in its current reports nos. 3/2012 of 20.01.2012, 13/2012 of  27.01.2012, 48/2012 of 02.05.2012, 63/2012 of 21.06.2012, 76/2012 of 08.08.2012, 93/2012 of  06.09.20112 100/2012 of 06.09.2012, 113/2012 of 22.11.2012, 118/2012 of 19.12.2012,4/2013 of 14.01.2013 and 24/2013 of 28.02.2013 and 31/2013 of 18.04.2013. As a result of signing the said settlement and  annexes concerning services rendered in 2012, the value of the contract with highest value has changed and  it currently amounts to PLN 11,920,580.20 (an increase by PLN 12,447.20).The total maximum Fund Branch liability towards  the Company for the healthcare services rendered by St. Anthony's Hospital in Ząbkowice  Śl.has changed and it  currently amounts to PLN  14,123,290.83 (a decrease by PLN  13,108.03) for the period 1 January to 31 December 2012. The other material terms of the contract with the highest value made public remain unchanged. A criterion for a contract to be considered significant: The value of contracts exceeds 10% of the Issuer’s equity capital.
