Report no. 12/2016

Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodical information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A., in connection with receiving as of February 5th, 2016 information that the value of the contracts signed by the subsidiary Mikulicz sp. z o.o. and the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch) has changed. The said contracts referred to healthcare services in the scope of nursing and care services in terms of the long-term care as well as primary healthcare for the period 1st January to 31st December 2015. Healthcare services referred to hereinabove are provided by the subsidiary Mikulicz Sp. z o.o. in Świebodzice owned by EMC Instytut Medyczny S.A.

The Issuer informed about the contracts concluded by Mikulicz sp. z o.o. with the Fund Branch concerning rendering healthcare services in 2015 in its currents report nos. 32/2015 of 4.03.2015, 43/2015 of 24.04.2015, 47/2015 of 26.05.2015, 53/2015 of 07.07.2015 and 96/2015 of 28.12.2015, 4/2016 of 15.01.2016. As a result of signing the said annexes to the contracts with the National Health Fund regarding healthcare services provided and the payment for primary health care services in the period 1st January to 31st December 2015, the value of the contract with highest value has not changed and it currently amounts to PLN 10,964,511.40.The total maximum Fund Branch liability towards the subsidiary company currently amounts to PLN 15,072,269.29 (an increase by PLN 366,370.29) in the period 1st January to 31st December 2015. A criterion for a contract to be considered significant: The value of contracts exceeds 10% of the Issuer’s equity capital.
