Report no. 25/2012

Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodical information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. informs that on 15 February 2012 the Company received an annex  the contract concluded with the National Health Fund - the Opolskie Voivodeship Branch in Opole (the Fund Branch), regarding healthcare services in the scope of outpatient specialist care for  the period from 1 January to 31 December 2012.

Healthcare services referred hereinabove are provided by the hospital owned by EMC Instytut Medyczny SA - St. Roch's Hospital in Ozimek near Opole.

The Issuer informed about the contracts concluded with the Fund Branch concerning rendering services in 2012 in its current reports nos. 7/2012 of 24.01.2012 and 18/2012 of 30.01.2012

As a result of signing the said annex, the total maximum Fund Branch liability towards the Company has changed and it currently amounts to PLN 13,282,764.48 (a decrease by PLN. 41,742.90) for the period from 1 January to 31 December 2012.

The other material terms of the contract with the highest value made public remain unchanged.
