Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodic information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. reports that on 17 December 2014 the Company received  an annex to the contract concluded with the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch), regarding inpatient  care health programs (Prescription Drug Program) for the period 1 January to 31 December 2014. Healthcare services specified hereinabove are provided by “EuroMediCare” Specialist Hospital with the Outpatient Clinic in Wrocław owned by EMC Instytut Medyczny S.A. The Issuer informed about the contracts concluded with the Fund Branch concerning services rendered by “EuroMediCare” Specialist Hospital and the Outpatient Clinic in Wrocław in 2014 in its current reports nos. 13/2014 of 25.01.2014, 22/2014 of 02.05.2014, 91/2014 of 30.10.2014 and 106/2014 of 15.12.2014. As a result of signing the  abovementioned annex, the value of the contract with the highest value has not changed.  The total Fund Branch liability towards  the Issuer with regard to healthcare services provided by “EuroMediCare” Specialist Hospital with the Outpatient Clinic in Wrocław currently amounts to PLN 13,715,177.70 (an increase by PLN  26,168.00) for the period 1 January to 31 December 2014. A criterion for a contract to be considered significant: The value of contracts exceeds 10% of the Issuer’s equity capital.
